This portal is for the exclusive use of Arizona Musicfest donors to upload their Proof of Vaccination documentation for entry to Arizona Musicfest performances during the 2021/22 Season.

Only a copy of the following documentation will be accepted through this portal:

Physical vaccination card

Official Immunization record

The above two items are the only acceptable items for this portal. Once your documentation has been uploaded and reviewed, a VAXPASS card will be issued and mailed to you along with your Donor Welcome Packet on or around October 15.
