This portal is for the exclusive use of Arizona Musicfest volunteers to upload their Proof of Vaccination documentation for entry to Arizona Musicfest performances during the 2021/22 Season.

Only a copy of the following documentation will be accepted through this portal:

  • Physical vaccination card
  • Official Immunization record

Once the above documentation has been uploaded, please allow a week for verification. You will receive a confirmation email upon submission, and another verifying that we have reviewed your documents. Upon arrival at the concert venue, you will be issued a VaxPass Lanyard for that evening.
